Fallout 3 play-through ideas?

Ok so I've Played Fallout 3 since it came out, literally over several thousand hours amongst all of my characters. Do any of you have cool ideas to freshen it up? I tried about everything, you die=it's over, only heal at home, no fast traveling. Just to name a few. What all have you guys come up with?

what about only  using the machete? or using stealth kills only? try like i dont know going rambo on the camps..... or maybe just try to always be drunk

I used a dope fiend character once, too expensive. Just like real life lol.

That actually sounds difficult, I don't know how I'd manage to kill anything. Maybe I could do a "no kill" character with naked + fists?
