Make sure it isn't clogged with dust or any obstacles blocking the fan from spinning

Are you sure it's at max speed? Have you checked what RPM it is running at exactly? For some cards, it's normal to see one fan spin while the other ones remain at 0RPM.

 

You have to see what model card you have and what is the expected fan behavior. Your core temp might not be why that fan is spinning, it's probably doing that based on some other measures.

 

You have to monitor and see what happens under load, and if your temps are normal and all fans spin, then leave it be.
