Strange graphic glitches

Like a month ago, I started to get weird graphic glitches while gaming.

It seems to start completely random, I could be gaming for the whole day without anything, or it occurs after a few minutes after starting a game.

I randomly get a black screen for a few seconds and when the image returns, everything is pixelated, when I turn the monitor off and on again, it seems to get no signal, but I still hear the sounds.

When this problem appears, the only thing I can do is a hard reboot.

I've recently noticed some new problems. When playing Red Dead 2 today, some shadows were flickering or the distant fog changed colors when I was moving the cam.

I've built this pc not that long ago, like two or three months.

A week ago I've connected another monitor and when the problem occurred, I've turned off the pixelated monitor and as I turned it on again, both screens turned black and could not find a signal.
