The reason for the 3 pin females is so that you don't have 3 fans trying to report speed to the same header on the motherboard, giving bad numbers. The missing wire and pin is the RPM speed signal.
It does not matter which one is plugged into the 4 pin female.
Yes 4 pin fans are PWM.
Yes PWM splitters will let more than one PWM fan be controlled. You can even use a set of regular 3 pin fans in a 4 pin header as long as they are ALL 3 pin and the header is in DC mode and not PWM.
