Color Banding Problem

Hi i have been facing this color banding problem like in attached images, this weird pain effect like problem is disturbing me. It's been almost 2years scince the problem but didn't found the solution. I already posted one problem while viewing 360degree videos (clipping/screen tearing) issue but it didn't resolved yet.
The problem will appear when i restarted my system(sometimes) and sometime while it's running, it will go when i shutdown my system and turn on after some time.
My Config:

Win 10 pro (build 19042)

Intel 6700K

Msi-z170 Gaming M5

Ram 32GB (2*16) (Corsair Vengeance CL16 2400Mhz)

Nvidia GTX 960 2GB(MSI)

SSD 250GB(Samsung)

HDD 2*500GB(Segate)

HDD 1TB(Segate)

Monitor DELL 21.5" (HDMI)

 

I am uploading my Display info in txt file

 

the thing i did to solve the problem(but no use):

1. Completely uninstalled the GPU drivers and reinstalled the latest drivers
2. checked for loose power supply connections 
3. completely removed GPU and used On-Board Graphics
4. changed the monitors and cables
5. changed the type of connections HDMI/Dp
6. Reinstalled OS

 

I am VFX artist while rendering images i am getting screen tearing like effect i posted my problem in those VFX forums no solution.
so please please please help me.
