windows 10 doesnt change resolution

Windows 10 seems to only have the ability to SCALE full screen apps to whatever resolution the desktop is running at while rendering the app at a lower res , but doesn't actually change the resolution the monitor is actually displaying.

How do you set it to output the correct resolution the app is set to rather than simply scaling it?

Only solution i found is to manually change the desktop resolution to match what the fullscreen apps resolution will be prior to running it.

I think this is something you need to set in the driver, e.g. for Nvidia there should be a setting like "use integer scaling", meaning the GPU does the scaling, rather than the monitor's resolution being changed. If you don't want that maybe there's an option to disable it and let the monitor change its resolution instead.
