Hey i noticed that when I'm playing with games or using and app and I try to switch to the recent app in alt+tab its switch to a random tab not the recent one and its so irritating

Maybe it's a background task that has a phantom window?

 

I've never had an issue like that before.

Microsoft has acknowledged the issue and has fixed it an in Windows Build 20231. 

So right now the fix is in beta but should be available soon I hope. it's really bothering me too.
