Yes I believe the second picture should cool off the fins themselves better. It doesn't look like the fan on the heatsink does much at all imo because typically they are mounted on the side. What I have configured is my 200mm fan on the side panel of my case is blowing air into and on the graphics card and my Hyper 212 Evo heatsink. Then the fan on my heatsink is blowing the air out of my case through the back mounting area for a 120mm fan. So in this case I pretty much threw one of my fans out. But aside from what I have done, as long as you have at least 1 exhaust fan then you should be good. If anything since the top exhaust fan and the 120mm fan to the left of your heatsink are so close, you can probably get rid of the fan that is attached to the top of your heatsink.

Just reverse your airflow for your case. You may opt  to set most of the fans to blow air in and create positive airflow but make sure you still have at least one fan blowing air out. Though I do not recommend doing this cause it make cause your components to accumulate dust faster.

 

Also based on my experience air cooling everything on my system its a matter of 1 or 2 degrees between blowing air in and sucking it out and I believe that's within margin of error and therefore negligible.

 

Though I still prefer negative pressure set ups. :) 
I'm using an Aerocool Mechatron 
with 2 200mm Bit Fenix Spectre Pros sucking air out at the top and side of my case

a 200 mm aerocool stock fan blowing air in at the front

140 mm  Bit Fenix Spectre Pro blowing air in the bottom 

120mm  Bit Fenix Spectre Pro Ghetto mounted inside the case to direct airflow from the 2.5 drive cage to my GPU

and another 120mm  Bit Fenix Spectre Pro sucking air out air at the back of my case. :)
