My computer crashed after screen glitch

I have frequent computer restart after screen glitched like the uploaded image. The glitch usually occur during youtube playback and resulted in the screen glitched for 2-5 sec and then the computer restart by itself. It started to occur after around ~3 hours youtube playback time.

I have done several thing to try fix it such as update Bios, Fresh Install windows, update my Nvidia experience, reinstall and update firefox / chrome. But non of these help with the crashes.

Anyone have an idea what I am dealing with?

Edit:

Currently running with following spec

Intel i7-4790k, Nvidia GTX 970, MSI Mobo Z97 Gaming 5, 2x8 Ram DDR3, 250 SSD Sata III

Using following version of software

Check event viewer in windows. From the picture it looks like GPU is crashing, could be faulty. Should be the driver crashing. Could you list your PC specs?
