Stutter even on desktop

This has been happening since updating to Windows version 2004 with all optional features (screenshot). It not only happens with audio, I just thought that made it easier to spot than looking at my mouse lagging. All temps are fine, Ryzen 5 3600 @4Ghz 1,3V and Rtx 2060 both lower than 50 in idle.Enough ram 16gb and at stock speed. I checked for malicious background programs using taskmanager and that weird taskmanager+ (what I call it)  you can download from microsoft. After a restart it seems fine, still stutters but a lot less often. Ingame the screen sometimes pauses for up to 5 seconds than everything runs smoothly again

If you boot to any other environment, does it still do this? I'd try booting to a portable bootable thing like a WinPE environment or Hiren's.
