Starting a few weeks ago my USB ports would occasionally disconnect and reconnect.  A few days ago Bluetooth stopped working and shortly thereafter all USB ports started disconnecting and reconnecting over and over before stopping completely.  Restarting machine led to same issue but the USB ports failed quicker second time.

The computer was built January 2019, and in December the motherboard failed completely for unknown reason and was replaced by same unit, so I assume the issue is mobo or PSU related again.

Anyone have any other thoughts what might be the cause?  If I replace the mobo or PSU any recommendations for this particular build? (I will send in to ASRock for another replacement but it seems foolish to pair this PSU and mobo and third time.)
