Vega 64 AIO LC Constant Crash Under Load

I've had an AIO cooled Vega 64 for a few months now and have infrequently seen crashes in games when settings are cranked too high.

Especially now this issue is prevalent because I have overclocked my LG 25um58-p to 75Hz.

When I use V-Sync so FPS can't go past 75, the GPU crashes after playing for a few minutes under high load.

This issue is most common on Forza Horizon 4 at 1440p with everything maxed. FPS is non stop at 75 with no dips and the GPU activity hovers between 70%-100% with the temp at 45c-55c at 2500rpm.

The fan profile is the only setting i have changed in the AMD Radeon Settings.

 

Does anyone know a fix? I know its not my PSU because the issue is identical on my 550w PSU (current one is 850W) and my Xeon E3-1231 V3 barley gets past 30c under full load on a CoolerMaster Nepton 240m?

Have you tried DDUing and reinstalling drivers?
