What is the best suitable graphics card I can pair up with my Core 2 Quad Q8200?

I currently have 4GB RAM (upgrading to 8GB RAM), 320GB HDD (upgrading to an 240GB SSD).

 

Something that is good at playing QHD content and possibly even 4K content!

That C2Q 8200 has to go I'm afraid, it's just too old these days.
