The R in RSI stands for repititice, so you don't want to do a certain motion a ton, especially not a bad one.

When you have two sorts of pointer input devices you can simply change in-between them to minimize the repetitiveness of using such a mouse.

 

Otherwise which is more comfortable depends on your personal preference, but also the duration of use and the kind of use (maybe one solution is better to use for shirt term PC usage, the other for more long term).

Logitech came out with the MX Ergo PLUS trackball, which gives you both.  There are two SKU's, the PLUS version comes with an extra base to make it more vertical.  The regular SKU does not come with the base.  (but both are the same price for some reason)
