I have a decent gaming pc which runs things like apex at max settings, 100 fps (1080p) but it can't run Minecraft without lag spikes. GPU usage is 0% until I blow something up and then it increases for a split second but the thing takes way longer to render. Very confusing.

I have

Ryzen 5 2600

16GB 3000MHz RAM

RX 580 8GB GDDR5 VRAM

500GB SSD (200 spare)

Any tips on how to stop this? It should be running really well and it's infuriating that it runs Minecraft worse than a 2010 iMac

What does your cpu usage look like in game?

Do you have the newest 64-bit JDK installed? (JDKs in general often seem to work better than SDK). If you don't specifically need to be playing the java version of Minecraft, consider switching to Bedrock/Windows 10 Edition, available from the Microsoft Store. You can crab a copy for free from Mojang if your account is older than six months (October 2018).

In my case it happened to me. I have 8gb 2400 ram, i7 7700HQ and a gtx 1050. What I found useful is when I launch minecraft I right click and select run with graphics processor then High-performance NVIDIA processor. That seems to do the job for me as modded minecraft didn't run well and I found out it was using the integrated graphics
