I run PFSense, but haven't had to mess with too many things other than pointing to my Minecraft server. Everything I've played since setting it up seems to be happy without my babying it (Planetside 2, Smite, MWO).

One thing that would help narrow things down is whether or not you have your modem in bridged mode. If it is not bridged the modem could be enforcing its own NAT rules prior to it getting to the firewall. You can bridge it to ensure its not interfering or disable any NAT or firewall settings on the modem and let PFSense do its job.
