SDD Dissapeared

So when I was installing a program I tried to install it to my 2nd SSD. It said file could not be found. So I restarted my PC and now it's missing in windows. I went into the disk formatter and it says to intilize it. So I did but it fails and says file not found. Did my SSD die? It's a 850 evo and last checked it's health was great
