Help with port forwarding

I don't quite understand my routers port forwarding GUI.

I need the settings from the first screenshot to be in the settings in the second screenshot. I'm trying to forward to port 1604 from Server IP Address 192.168.1.3

Can anyone help?

App Name: Whatever you want to name it

Protocol: I think you should be fine at TCP/UDP

Global Port Range: 1604-1604

Local Base Port: 1604

Select Device: use the drop down menu to select your device at 192.168.1.3
