If the GPU freezes at stock frequencies it could probably be another problem

Card might be freaking out because of VRM/MOSFET's over heating. If it needs to downclocked try opening the card up, reapplying thermal paste, checking on your VRM's and whatnot then applying non-conductive thermal pads to them.

That should help with the poor temps, assuming the card is COMPLETELY cleaned out I'm not sure what the issue could be aside from a weird voltage irregularity. If you have modular power supply you have more than 1 PCIE section to plug into, try plugging the power PCIE power connector on your card into a different PCIE power connector on the PSU.
