Monitor randomly disconnects

Recently my monitor has started randomly disconnecting. There is no pattern to it sometimes it will happen a few minutes after I turn my computer on and some times it will go hours before disconnecting. I have tried switching display port cables. I saw a solution that said reset your graphics card that did nothing.

 

Not sure what to do from here.
