Looking for summoners to join new clan in League of Legends for ranked 5's

Hi guys and girls.
My name is Jim. My summoner name is NerfDeez.
I currently play ranked solo que and ive been ranked between B5 and B2.
Im looking for other summoners to create a ranked 5's team with.
I would like to start soon to get some good practice before this season ends so that when season 5 kicks off we have a better shot of better ranked placement.

Requirements:
18 years old or older! Sorry but maturity is a must.
Desire to get better than you currently are. Striving to improve is necessary for success.
Ability to play on a regular basis. 3+ days per week.
Fun attitude, strong team work ethic, honorable in victory and defeat
Decent computer hardware and internet
No history of bans or chat restrictions

Also a plus:
Ability to skype
Commitment to a specific role. The more you play a specific role for a team the better the team can get.

Details i would like to see include the following:

Summoner Name
Current Solo que rank (i dont care if your b5 you can still join)
Preferred role, secondary role
Main champs used
Your strengths and weaknesses
Days and Hours (hours in est please) you usually able to play

Personally i like to play three roles.
Main role = mid. I like to play either morgana or heimerdinger mid
i am also in the process of playing more mid champs in normals to expand my champ pool.

Secondary role = support. I also play morgana as support as well as blitzcrank
again in the near future i will be practicing with other champs to expand my champ pool

Tertiary (3rd) role = top. I have only played heimerdinger top to this point. While other people hate when i do this it has mostly been very effective. (got me from B4 to B2)

When we start this clan i will adjust my champ pool and play style to fit with what we are trying to do as a team. Meaning if i need to learn a specific champ for a better team comp i will do that.

Final note. I would like to have a total of between 7 - 10 people in our clan so that we have enough people on given days to play. If we only have 5 or 6 we may not be able to play regularly enough to satisfy everyone in our clan.

Garen/Udyr/Ezreal/Ashe/Yi alot i know
I can get a first blood easy but im not good at knowing when to engage in team fights 
I can play from 10am to 5 pm

I know i'm not 18 but that doesn't make me immature
