Cross-Platform Friends App

Lets say there is an App for Xbox, PlayStation and PC. It lets me make friends with people on these other platforms. How could we get it to invite people into what ever game you are playing at the time?

 

A developers version of the app, they can install on their severs allowing this? Or a separate play type platform like steam?
