Audio jack mounting

I am sick of reaching around my PC to plug in the stuff I need, so I figured I'd mount an extension to under my table. But I haven't been able to find the right piece, maybe one of you guys can help.

 

It doesn't have to be anything fancy, I just want a simple plastic piece to mount under my table (preferrably a piece I can screw to the table), which I can plug into my PC and then easily access when sitting at my table.

got mine for 5€ so this one is the same but a higher price.

They are also aviable with usb 3.0 or without usb.
