I have a Sapphire 7970, for witch there is a documented issue when using a multi monitor setup and almost any OC software. The problem lies in crimson/afterburner applying your new clocks to the 3D profile but when the card runs 2D the core and mem are running at only 300mhz causing awful screen flicker and artifacting.

I am aware of GPU tweak II being able to force the 3d profile to always be on but does anyone have a suggestion for OC software that allows you to set a custom 2D AND 3D profile?

Try Sapphire's own TriXX software first.

Tried that, it was some really basic stuff, no mention of 2d or 3d. Monitoring my card indeed showed a trixx profile worked fine in games but would drop gpu core and mem to 300mhz causing the flicker.

I got infuriatingly close with ASUS GPU Tweak 1, in advanced mode it has the exact feature I want. You can click 3d, set clocks, switch to 2d set clocks. Only issue is the software is so old. No proper support for my gpu and the buggyness was a deal breaker although it worked for a short time fixing my flicker while allowing a custom overclock.
