Then you should probably reinstall the game.

 

Your CPU isn't the best but it should suffice for pushing 60FPS at High settings. I played BF1 on my friend's rig yesterday which has an old i7-870 from 2010 @stock and an RX 580 8GB, it played the game a lot better than your setup did so it might be the game.

Potentially, yes. But HDD related issues would be probably random stutter and not decreased performance overall. Are loading times unusual?

 

You can test your HDD just to be sure though.

CrystalDiskInfo might give you an indication about your HDD's status (ex. reallocated sectors count). You should run CrystalDiskMark as well and post screenshots here.

 

Remember to go into Function->Advanced Feature->Raw Values and check 10[HEX] so the results are human-readable (in CrystalDiskInfo).
