As mentioned above by @njmyers3, CrystalDiskInfo will tell you the exact model of the drive, as well as statistics such as health, power cycles and power-on hours.

CrystalDiskInfo will tell you if the drive is healthy or not. It will give you the amount of time it has been spinning and how many power cycles it has had.
