I use keepass since I can use it across all my computers Linux and Windows, plus it is free and open source.  http://keepass.info/  If you don't like the gui that comes by default someone made one built with JavaScript https://keeweb.info/

I have LastPass. Its extension for desktop browsers and paid service for mobile, but you can as well use website to check sites you don't remember.

Seems like LastPass can indeed store notes. It also has support for Googles 2FA, with option to store that with secure devices for 30-days.

 

LastPass has been hacked twice while I've used it. Both times they informed customers as soon as it was safe (you don't want anyone changing master password or email when someone might still be locked into system) and did reset for master passwords. And neither times hackers did get anything which could compromise account or saved sites. I think the worse was about login emails.

 

There's no 100% secure system. Keepass gets close as its available for outside influence only when USB is connected. But if you loose USB or is connected to infected system, there's a risk. Same risk goes with .txt files in password protected folders and using basic pen and paper.
