I have had a 950 PRO which my OS is installed on and a 850 EVO that I kept my games on. The 850 Evo is filling up, so I decided to put a second one in. I had used the one I put in on a previous rig. I can format the SSD, but the second I try to install something on it, I crash and am forced to restart. Occasionally the SSD does not even show up. I replaced both the power and sata cables, so they do not seem to be the issue? Any ideas?

Could be a bad SSD indeed.  Do you have access to another computer so that you can test it in that one?  Perhaps a 2.5" enclosure so that you can connect it via USB to your own PC or a laptop?

Another thing you could do is to

1 ) remove your current SSDs (so unplugging the 850 and removing the 950)

2 ) plug in the problematic SSD in the same SATA port you want to use it on

3 ) try to install Windows (I assume you're using Windows, based on your profile) on the problematic SSD.  Seeing as it's the only SSD that's connected, you don't have to worry about messing up your regular install.

If that install runs just fine, the problem isn't the SSD itself but something about the OS install that's on the 950.

If the install fails or crashes on a regular basis, wipe the SSD, plug it into a different SATA port and install Windows again.  That way you'll know if it's the SSD itself or a bad SATA port on your MoBo.
