Graphics Card driver Crashing...?

So today, as i was browsing chrome, I was experiencing lag with a page as i was scrolling through it. There was a youtube video embedded in the page playing, and the lag was continuing. and eventually, my monitor froze, my mouse disapeared, and both of my monitors went black for a moment and reapeared.

This happened again as i was loading a youtube video. Any ideas on my issue, what it is, and how can i fix it? Here are my specs:

Chrome is a memory hog and unoptimized on even new computers with DDR3/DDR4. Using it with a DDR2 system is pretty much guaranteed to have some issues.  And only 5GB. :P

It might help to disable flash and opt in for HTML 5 youtube.
