And I'd also physically check the wiring while the system is off since it's not hot-swappable I don't think.

Did you check to see if the USB front IO header was on the right spot or was it just plugged in? It might be in the wrong spot. Maybe front IO header is broken, try plugging the headphones into the back to verify it isn't the headphone cable not working
