R9 380 help please!

So for the past few Days my pc hasn't been acting right at all. I can hardly play games without my monitor going black and my periferals locking up. Right now I'm using Radeon Crimson 16.9.1, clean install. But before I've reverted back to 16.3 and even 15.11 and I'm still having issues and I'm at my wits end and I'm frustrated beyond belief. I have an XFX r9 380 DD OC XXX being powered by an EVGA 600b on an F2-a88xm motherboard with a 7870k OC @4.4ghz. What I've noticed is that when ever it freezes the gpu fans ramp up to max for a second before shutting down, and the only way to restart to system is to do a hard reset. I need help, please
