GPU failing?

My GPU won't draw more than 11.26 volts from the 12v rail. PC would shut itself off during games (DOOM, CS:GO, DotA, Dark Souls III...) because when I would return the clock speeds back to stock, it wouldn't shut itself down. Now it is shutting itself off after roughly 20 minutes of gaming. Any advice? I attached the Txt file from HWMonitor.
