VIP Access Security Issues

I use VIP Access as a two form authentication service for a variety of applications.
Apps on my mobile device and applications that use push notifications still function correctly.
The issue I am running into is the dynamic security code generated is displaying all zeros.

Applications that require the security code to be entered do not work. 
Has anyone here run into this issue who use this app? or maybe another two form authenticator?
