The 5k score looks like the card isn't doing much through it.  Download GPUz, and open the sensors tab and run it again.  Take a screenshot of the sensors tab right after you run firestrike and post it here.

The 12k graphics score is kinda low / average, about what I'd expect with stock clocks.
