Upgrades for my Desktop

HI!! I am looking for some graphics upgrades for my desktop system. I have a MSI DDR2 Motherboard, 4GB Ram, 1TB HDD and also a NVIDIA 210 DDR2 GPU(Believe me, I still have this). Could any one suggest me some graphics card upgrades for my Desktop. (I prefer AMD to NVIDIA). Graphics card which support latest gaming tiles at a playable frame rate (30FPS) are appreciated.

I apologize for my late response since I was unable access my Internet for few days. And my CPU is Core 2 duo E4600 clocked at 2.4GHZ

I would get a 950/750ti then, your CPU will bottleneck a R9 380 a lot.
