MIcro SD card problem

I have an USB micro SD card reader and Windows can't see my 16GB SD card. I tried putting it in my phone but even my phone won't recognize it.

 

#when I put the sd card in the USB reader and I plug it in, Windows says it must be formatted then I click 'ok' but it says It cant be formatted.

 

Any help pls?

Try to reformat it at other computer, if it still doesn't want to work then might as well get a new one.

Why not you try Lubuntu live CD to run:

 

df ->see which is your sd card

umount /dev/sda ->or /dev/sdc, whatever it is

sudo mkfs.vfat /dev/sda ->replace with whichever /dev/ it is

then try
