GPU crashes after while CPU and GPU are working hard

Hello

 

I really got a problem. 

I got this problem for a while now and first I thought it was motherboard issue... but it wasn't.

 

when me CPU and GPU are both under load me GPU will crash. what I mean with that is that the game will freeze for a second and after that will not give the same performance. I need to restart me pc in order to fix that.

 

when I'm playing bo3 (or any other game that is also high cpu hungry) it will freeze.

 

I have not OC me GPU but I did had a OC with me CPU even reseting to default settings (checking the ram speeds are okay) I still got this problem

OS:windows 10 (fresh install)

CPU: i7-4770k

GPU:gigabyte gtx960 g1 gaming

motherboard: asus z87-c

RAM: Corsair Vengeance 2x kit of 8g

PSU: sseasonic ms12 55wat

it may be a faulty card. Are all wires securley down and on the card and psu? Are the fans spinning when it gets hot?
