Disk usage at 100% for no reason. regular Task Manager doesn't say what is using up my disk.

Ran multiple hard drive utilities including but not limited to hard drive sentinel and they all said my hard drive was in pristine/perfect condition.Also ran memtest+ off of bootable media for 5 hours and it came up with 0 errors. Malwarebytes came across nothing either and no windows updates or maintenance is occuring,so what the actual fuck is up with my computer.

Someone please help. also even when the disk usage spikes down to 1%-10% every once in a while it's like task manager is lying to me because everything still happens slow and programs take forever to load ):

What are the specs of your system? And do you have proper cooling for the HHD? (At least a fan of any kind blowing on it this sometimes helps) (Unless you're using a laptop) Possibly get an SSD and transfer all of your data on the new storage device.

I want to check something - are you possibly confusing percent usage with percent of highest active time? Just as an example, if your CPU had been fluctuating around 0 - 10% used, any time it was using 10%, it would show 100% highest active time, since it is matching the highest load it's been under in the history of the chart. (I know there isn't actually this type of chart for CPU, just an example)

I'm thinking this because the transfer rates I'm seeing are not anywhere near what the disk should be capable of if it was pinned to 100%.
