Does anyone know if Nvidia GameWorks is always on or is it something that has to be downloaded?

It's built into the game settings if supported. You need a Nvidia graphics card to use them

It's something in the game that runs better on compatible Nvidia GPU's.

Gameworks is essentially a library of effects that developers can use. It is basically a developers super pro version of a "make your own game" program. It helps them create advanced and code heavy stuff faster then usual by using predefined code that you can just copy pasta and then tweak.

It runs better on Nvidia mostly because Gameworks leverages TESSELATION in quite a hefty way. Which is great for games and great for Nvidia which has been better at tesselation for a long time (this is why forcing lower then x64 tesselation in CCC helps AMD performance. Personally i cannot tell the difference of x64 vs x32 tesselation on my 1440p monitor).
