Volume Up, Home and Power buttons at the same time

 

 Power, Volume Down and Home buttons together.

vol up home and power

VOLUME UP button and POWER 

try pressing just these buttons to..if the rom manager doesn't work

someone said you don't have to press all 3

knox is a security software pre-installed on the latest updates for most Samsung phones include the gs4, and it prevents rooting and most changes to android if i remember correctly. so it might be the thing preventing you to install the recovery correctly

After you install the CM12 rom if it is stuck at the Samsung logo loading try doing a factory/cache wipe from the recovery. That helped me in certain cases. 

 

EDIT: I just installed CM12 on my S4 today.
