My friends PC is constantly running at 100% disk usage. He is running windows 8.1. i7 3770k, 16Gb RAM, Seagate 2TB barracuda. Whats wrong? I have tried all fixes on youtube and websites and no luck.

Check what's using the disk via Task manager?

Is it causing any problems? If it doesnt affect him then dont worry about it.

It is. he can barely run windows and playing games is at 1fps. So i think its causing problems :/

Is it causing any problems? If it doesnt affect him then dont worry about it.

Nothing shows up. Its running at idle. No applications are running.

My friend had this issue. We didn't know what to do, so he reinstalled the OS.

This didn't solve the issue, which was weird. Turned out the dummy let windows save his old files into "Windows.old"...

He reinstalled the OS again and that fixed it.

Pull up task manager, on the performance tab, click on Resource Monitor (bottom right corner).  Go to the disk tab.  From there you should be able to see what process is specifically using the most disk access.  How full is the drive?  Any virus scanners?

Follow what @ChineseChef suggested and see if the drive is used by something. Also I would suggest running a diagnostic utility to check the drive's health. I would use the manufacturer's one as it knows the firmware best. 
Checking for malware and if you have the latest updates is also a good idea. Also see if your antivirus program is not performing any continuous scans and thus slowing your drive down.

I run a 2TB Seagate barracuda, and the anwser is simply, that Windows eats the harddrive, and when it doesnt, Steam does, and if none of those doesnt, well then your antivirus does. if you dont belive me check out the performance watching thing in windows and see what processes does it.

Either Windows is running a background defragmentation or it's SuperFetch. Try disabling superfetch for a moment and see what happens. It's a common issue with Win8.

Can you try the drive on another computer to see how it performs? Possibly another computer that already has a bootable drive and this one would be only storage one? Did you try the manufacturer's diagnostic tool?

If you are able to see that it is at 100% utilization, there should be something showing what is using it.  In the resource monitor, what was showing as using the most disk time?
