Help installing SSD disk read error

Im going to start from the beginning. I got an SSD (Patriot Blaze 2.5 sata III 6gbps 120 GB) and tried to put a new install of windows 7. So I pluged it into my computer and unpluged the old HDD and pluged in a USB DVD reader with win7 disk. I ran into problems with the usb not working(drivers) but just pluged it into a different usb slot. It started compying the files then its shut down for the first re-set. Heres the problem. I selected it to install to the SSD which it did(took up memory look at picture that's the SSD) but when it tires to boot from it its says "disk read error" so i googled to see the problem and they said it might be broke, so i plugged it into the computer and looked at it. also ran Crystal Disk Info and it says the SSD is in good condition. and i can use it but not as a boot drive. I switch sata cables and the ones i used also work with the HDD so this can not be the problem. same for the power.

 

Mobo: 970A-UD3 ATX AM3+ (don't know bios version) 

SSD: Patriot Blaze 2.5 sata III 6gbps 120 GB

old HDD: WD 2tb green?

os: win7 home p 64

 

How I installed Win 7:

everything plugged in unplug old HDD

start computer select boot drive(f12) select usb-cd/dvd

in windows launcher click install custom, click SSD, click format, click next starts to go

computer re-sets

computer boots to SSD and i get the error "disk read error C+A+D to reset" 

The first time I installed win7 I used this method same disk different USB reader(but it reads movies so it seems to work)

 

I tired to install win 7 muti times this way

also changed the default boot in bios to start usb-cd/dvd then change it again after reset to SSD

also tried usb but it didnt work said there was nothing on usb(maybe too small its only 4gb)

Any help would be great had the SSD for around 3 weeks and will need to return it if it is broken within the 30 days.

I know you can clone win7 form the old HDD and put it onto the SSD(I don't know how too yet) but i read that's its better to do a clean install but I might have to go this way.

 

conclusion: it seems that the SSD works(i can store stuff on it) but cant use it as a boot drive because of the error. I mainly want it for a boot drive and 1 or 2 games

So today I tried to install win 7 to a new SSD and when installing it I got an error. The first one was there was no dvd driver but that went away. than when installing windows 7 it got to 70%+ and it restarted and said  "loading operating system... a disk read error occurred" so i looked quickly and read that it might be broken so I check if I can see it on another computer and I can. 

so what could be the problem?

 

Stupid but necessary question: is the SSD empty?

 

I'd recommend running a utility to test said SSD if possible. What brand/model/capacity SSD is it?

it was empty before trying to install the windows 7. 

brand: patriot

model blaze sata 3 6Gbps

size: 120gb

 

crystaldiskinfo says its in good condition

Did you install this from a disk drive? if so, it was probably an issue with the disk or drive itself.

I am trying to install it with a usb dvd reader. This is how I did it the first time I installed windows(not the same usb dvd reader though) but I check the dvd reader to make sure it worked and it played a movie just fine

It was probably the reader then. Or it could be the disk, it only throws that error when reading from the install media. It could have also been the USB connection itself.

Ok guys i updated my post with a new problem/same problem that I am having.

Please help guys I updated my post with everything I could think of. From what steps I took to install it and what parts I changed.
