ssd problems -_-

I have a ocx vertex 3 max ipos 220 gb, and im seeing some problems. First when it starts up it goes smooth then it screws me over and after I logged in it froze. 2nd every time i use a type of internet usage with a application like chrome and league or any other application that dose not use internet, If i was using chrome and league was in the back ground both of the application stops running and freeze for 10 sec or more and if i try ctrl + alt + del if goes into that screen but it stops from there it say something and it keeps on loading for more that 20 sec then i had to restart. 3rd if my computer freeze i restart it and it turns into a black screen.

 

 

is there a update or a software to resolve this problem

It may be dieing, but if you recently got it that is less likely. (I bought a "brand new" ssd from amazon, it came in without original packaging, in a clearly damaged state, stopped working in less than 2 weeks.)
