Twitch TV - How to stream two screens?

Hey guys, basically I'm planning to livestream a few lan games of DOOM with my friend. I want to add both of our screens into the stream, but I don't know how to do this. I have a capture card if that is a way of doing it, or maybe there is some other way I'm missing?

 

If I haven't explained what I'm trying to do clearly enough just say

I do I have a HD PVR, how could I use it for this? Use a HDMI to component wire?

When we live streamed some stuff from the office we sent video streams via VLC over the local network to one of the PCs that then picked up the streams and compiled them into a single stream with OBS... worked out pretty well.
