It's getting hot in here.... your PC must be choked or something... Power doesn't go through enough.. PSU's overheating and it shuts itself off to cool itself down.. else it'll blow up your PC, with your CPU, RAM, Motherboard and whatever it is connected to.

you will need to open up your PC

do a cleaning routine and remove any dust in the cooler and fans

Budget? Uses? Currency? Location? Operating System? Peripherals? Monitor? Use PCPartPicker wherever possible.

From what I read from jonnyguru, 4v on the +5v rail is out of spec... Your 12v also looks like it barely makes the atx spec... I can think that it's a miracle that your PC boots at all...

EDIT:

I looked at mine and realized that my mobo has weird reading offsets so I cant exactly compare my readings to yours...

Well, it might be better to replace it before it reaches that point...

The lemon list has the PSUs that will die early or kill your PC early (including burning your house down along with your PC).... Going with a beefier PSU (450W-600W) will allow some headroom for overclocks, just avoid some of the questionable ones (like the 700W one I got for less than 20USD)... I suggest something by Corsair, FSP and CoolerMaster (refer to list).. You can also check out johnnyGuru for reviews on any of the power supplies you may have some interest in...

Also, I'll give aida64 a shot...

EDIT:

Just looked at aida64 page... 34.99USD for a hardware monitor!? are you kidding me!?

EDIT2:

Looked at aida64... Values match those provided by BIOS... but all the more important bits are censored...
