I've recently bought Fallout New Vegas from the steam summer sale and decided to try it out. When I first ran it, it recognised my gpu and set the setting to ultra. After seeing this I added some mods to make the game look better, but now whenever I run the game it uses my Intel integrated graphics instead of my GT 650M. I've tried going to the Nvidia Control Panel and changing it there but it doesn't help. Please Help.

It says Intel® HD Graphics 4000 and thanks for responding.

When you initially launch the game you should be presented with a small window with a few selections. Choose Options, and click on the dropdown box for 'Graphics Adapter'

Is the option for GT 650M missing altogether, or is it simply not selected?

It is missing all together.

Are your NVIDIA drivers reasonably up-to-date?

Perhaps try these ones (for 64-bit operating systems):  http://www.geforce.com/drivers/results/75992

Also, are you having an issue for just Fallout: NV, or have you noticed this in other games?

What is the model name of your laptop? Does it have a 'switch' that you can toggle to change from Integrated graphics to your discrete GPU?

Also, try going into your Documents folder > My Games > FalloutNV > FalloutPrefs.ini

Find 'sD3DDevice' and in between the quotes, change whatever is in there to read NVIDIA GeForce GT 650M

Like so:

sD3DDevice="NVIDIA GeForce GT 650M"

Save the file, and try loading the game again.

If not, I'd disable all the mods you installed, run the game and see if it works. If so, then re-enable one mod at a time  (enable mod, run game. Enable mod, run game, etc...)

It may sound tedious but if you have time I'd do that, might be one of the mods you're using, especially if it's to 'enhance performance' or 'fix in-game bugs and glitches'

Does the game actually run terribly, or is it adequate?

Fallout 3/NV was known to sometimes confuse itself between a discrete GPU and Intel HD Graphics 4000 (it would say the adapter was Intel HD Graphics 4000, but it was actually the discrete GPU running, not the integrated graphics).

Sorry if I wasn't much help =/ Good luck!

You should be able to force your laptop to use the GT 650M. There should be an option to do so within nVidia Control Panel.

My brother's laptop has both Intel HD 4000 and a GT 740M, and he's able to switch between them without much issues...if the program / game doesn't automatically detects the 740M and switches.
