Without V-sync on games are choppy and the screen is tearing with freesync enabled.

So, I have been trying to figure this out for a long time now, and that is why other people can play without v-sync and have a smooth experience and when I try to do this its choppy and tearing. I don't understand, I can turn vsync off and get 180+fps in a lot of games but its like playing on lower than 60hz or something. I also wanted to know if a monitor can cause stuttering? because I recently switched back to an older monitor and I get no stutters what so ever. Just to let everyone know I have freesync enabled.
