What drivers are you using?  Make sure you use all vendor supplied drivers, not the stuff windows installed. Also make sure to turn off driver updates on windows update. 

Verify the GPU power cables are firmly attached to the card and the psu. You are using 2 dedicated power cables correct?  Not the cable with the built in splitter?
