Im having a phantom gpu error

This error has been happening for months now where my gpu becomes very unstable and flashes black screens and says that ive disconnected it even at times reporting live kernel errors. Thing is this only happens when windows seems to not properly shut down when it happens and it wont stop until I do a fresh install of windows till it happens again inevitably. Im at my wits end with this issue as ive tried to update my bios, reset my cmos battery, uninstall and install the drivers, check for any damages on the card and theres nothing that is giving me an answer to my problem.
