Troubleshooting SSD

I built this pc earlier this year, but as of last week something new has happened to me twice now. My C drive gets pegged to 100% with very low read/write speeds and very high latency. 

 

The first occurrence was after the pc had been on and being used fine all day, and later in the evening I went to update a game on Steam and everything screeched to a halt. I was talking to my friend on discord while it happened, so it was as though anything that was already loaded and running still worked but extremely slow. I ended up resolving the problem by shutting down, opening the case and reconnecting the data and power connections to the drive; when I booted again everything was working great.

 

The second occurrence was this morning, I turned on my pc and after loading Windows, the drive was pegged at 100 and everything was slow. I turned the pc off, reconnected cables, and turned back on. It seemed to take a bit longer to load things than normal but the problem again went away.

 

I ran a test on my C drive (OS drive) and on my D drive in CrystalMark, and I noticed my read/write speeds were low on the C drive but fine on the D drive. At this point I am not sure if the drive is starting to fail/ I need to replace sata cables/ or something else is the problem. I will also say that while everything in this build was new, the SSD in question was the OS drive in my previous pc, so I also think maybe the fact that it wasn't a new Windows install is not helping?
